I ride at night every work day on the ride home (13 miles).
Schmidt SON28 and a supernova E3 for headlight.
four bright flashers on back, two flashers on each side.
Schwalbe reflective tires, loads of reflective tape and a reflective helmet.

You can see me flashing more than a mile away and the Supernova E3 accommodates 15+ miles an hour without reservation.
I used to run two E3's, one aimed high, one aimed low, now that was a sweet setup but I had to give one to my girlfriend.
The E3's are bright enough to annoy cars and cause them to flash you!
